If you have more than one child being dedicated, please fill out the form accordingly. If you are dedicating only one child, skip the “2nd Child’s Information” and “3rd Child’s Information” sections and continue filling out the rest of the form.
The church requests that you provide a picture of your child for the PowerPoint slide we put up while you are dedicating your child. If you so choose, please email the picture to the church’s administrative assistant: office@gospelcentre.com
